Diagnosis
After performing the initial X-ray a main duct “disappears” in the area of the middle third.
Treatment
The absence of the main duct in the area of the middle third suggests the presence of a Vertucci type V bifurcation.
To carry out the treatment in this type of cases, it is necessary to instrument with flexible files and without shape memory.
Files that allow pre-bending to know which canal is being instrumented are the best option. It is also necessary to preserve most of the tooth structure, especially in the coronal area, as it is key to prognosis. In this case, files with small and conservative diameters are the best option.

To finish the treatment, the thermoplastic filling is carried out with the Z-Down and Z-Up system.
