Dr. Álvaro Barrios, an endodontic specialist, received a 66-year-old patient at his clinic who was referred by another endodontist colleague. The patient had undergone an endodontic treatment three years ago, but the lingual canal could not be accessed during that procedure.
After evaluating the situation, both the option of endodontic retreatment and the possibility of performing an apical surgery were presented to the patient. After a detailed discussion, both the patient and the doctor decided to opt for the surgical option.

Surgical access
During the apical surgery, a surgical access was made to locate the lingual canal and the isthmus that connected the two canals (the vestibular canal, already treated, and the untreated lingual canal). Preparation was carried out in the foramens of both canals, and they were successfully joined by preparing the isthmus, allowing proper access for treatment.
Once the canal preparation was completed, obturation was performed using Zarc’s NeoPutty bioceramic cement. This bioceramic material offers bioactive properties and is particularly suitable for endodontic surgery.

NeoPutty was properly applied in the prepared canal, ensuring a tight and stable obturation.
To promote the bone regeneration process, bone grafting was performed using DM bone, a bone graft material that promotes the formation of new bone tissue.

In addition, a collagen membrane was placed to protect the bone graft and prevent the invasion of unwanted tissue at the surgical site. This collagen membrane provides a physical barrier that promotes proper and controlled tissue regeneration.

These advances in endodontics allow for greater precision in surgical treatments and promote better regeneration and recovery of the affected tissue.
