Reendodontics of a 1.6 with RetreatAll® and BlueShaper® file systems
Dr. Rubén Monge shows us in this case a patient with severe apical abscess in 1.6 with a previously performed root canal treatment. In the retreatment he uses RetreatAll® to remove the gutta percha and BlueShaper® for the instrumentation.
Case report: Root canal treatment of a 1.6 with previously treated acute apical abscess.
Diagnosis
The patient presented with continuous pain of medium intensity in the area, increased when chewing food of a certain hardness. During the clinical examination the tooth presented vestibular fistula at the height of the 1.6 and great pain on percussion.
The periapical X-ray showed a radiolucent area in the apical part of the roots. The CBCT confirms a deficient seal of the previous root canal treatment, as well as the existence of a second untreated mesiovestibular canal.
Treatment
Dr. Monge starts the retreatment of mesiovestibular, distovestibular and palatal canals with the RetreatAll® system. He first uses the 21mm ZR1 30/04 file at 500rpm to remove the gutta percha from the coronal third. Next, the ZR2 25/04 25mm file is used to remove the remaining gutta percha in the most apical part of the canals.
Once the gutta percha has been removed, he instruments the canals with the BlueShaper® Z1, Z2, Z3 and Z4 system for the 3 canals, in addition to locating and instrumenting the untreated MV2.
Before obturation of the canals, he activates the hypochlorite (5.25%) and EDTA (17%) with the Z-Activator® ultrasonic activator and its flexible Z-Blue tip.
The four canals were sealed with thermoplastic gutta percha.
