In this clinical case, Dr. Adrián Hernández shows a endodoncy of a tooth 35 with irreversible pulpitis, which also has a bifurcation in the middle third. Case report: Treatment of irreversible pulpitis in premolar 35 with bifurcation in the middle third.
Diagnosis
The patient comes to the clinic with irreversible pulpitis in premolar 35. Mandibular premolars are often complex to treat endodontically due to the wide variety of canal systems that can be present in their internal anatomy.
Treatment
In the preoperative X-ray we can observe the presence of a bifurcation in the middle third of the canal system, which in the Vertucci classification corresponds to a type V.For the instrumentation of this complex premolar 35, the doctor chose the innovative and revolutionary SlimShaper® system which, with its triple alloy, allows for efficient, safe and minimally invasive instrumentation in the pericervical region, respecting the original anatomy of the canal system along its entire length.
Both premolar canals were instrumented with SlimShaper® ZS1, ZS2 and ZS3 with the protocol recommended by Zarc4endo (500 RPM and torque 3).During the irrigation phase, 5% sodium hypochlorite was used by positive pressure with the Z-Rinse® G30 irrigation needle, ultrasonically activated for 30" with Z-Activator®. He also used EDTA 17% with 30" of activation, accompanied by a final irrigation with sodium hypochlorite 5%.
